All available question types for custom forms — including multiple choice and rating scales.
Short text, Paragraph, Email, Phone, Date, Dropdown, Single choice, Multiple choice, Rating scale, Signature, and Acknowledgement.
For Dropdown, Single choice, and Multiple choice: enter one option per line in the options textarea. Commas and spaces within option text are fully supported.
Set min/max values (e.g. 1–5) and optional labels (e.g. "Not at all" / "Nearly every day"). Respondents click a number button to answer.
Contracts and waivers open with a full editor — the same one as waiver templates — so headings, bold, italic and numbered or bulleted lists all work. Paste your agreement in and it keeps its shape on the public form, on the signing page, and in the printed copy. (On other form types the same editor holds a shorter intro.) Text written before the editor existed keeps its line breaks and loads straight into it, so nothing needs re-typing.
The signature box is not one of the fields in the list — it is added by the "Require signature" checkbox at the top, and it always sits last. It is shown in the field list as a greyed-out row so you can see it, with a Remove button that simply unticks the checkbox. If a field above ALSO asks for the signer's name, they will be asked for it twice: delete that field, or remove the signature box, depending on which one you want.
