Form field types and options

All available question types for custom forms — including multiple choice and rating scales.

Available field types

Short text, Paragraph, Email, Phone, Date, Dropdown, Single choice, Multiple choice, Rating scale, Signature, and Acknowledgement.

Adding options for choice fields

For Dropdown, Single choice, and Multiple choice: enter one option per line in the options textarea. Commas and spaces within option text are fully supported.

Rating scale

Set min/max values (e.g. 1–5) and optional labels (e.g. "Not at all" / "Nearly every day"). Respondents click a number button to answer.

Tips
  • • Rating scales work great for wellness intake forms — e.g. PHQ-9 style questionnaires.
  • • Use "Acknowledgement" for terms or waivers the respondent must check to agree.
  • • Building an instructor contract? Click "+ Contract template" in the field list to add nine ready-made fields — Effective Date, Signee Name, Business Title, Signee Mailing Address, Term of Agreement, Monthly Membership Fee, Contracted Monthly Hours, Security Deposit Amount, and Security Deposit Status (Paid / Unpaid / Waived dropdown) — in one click. These then become pre-fillable from the Send-for-signing dialog.
