View a member's submitted forms

A member's intake / Body Therapy / custom form submissions appear on their profile for easy reference.

Open the member profile

Members → click a member. Scroll to the "Submitted forms" card.

Read a submission

Each form the member submitted is listed with its date and a "Signed" badge where applicable. Click one to expand and read every field and answer.

Tips
  • • Forms a person submitted before they had an account also show here — they're matched by email as well as account.
  • • To send this person a waiver or form to sign, use the "Send a form to sign" card just above — it lists every waiver and form, each with its own Send button.
